Cancellation Policies

Care Ultima offers a 30 day cancellation period.

Member shall receive a reimbursement of all periodic membership fees if membership is cancelled within the first 30 days after the effective date. 

UT, AR and TN residents, if an application fee applies: A refund of all fees will be issued if membership is cancelled within the first 30 days.

MD Residents, if an application fee over $5.00 applies: The membership fee and one-time application fee (minus $5.00) will be refunded if cancelled within the first 30 days and upon return of the discount card.


Cancellation Instructions:

Email instructions: Send an email to info@careultima.com and provide the members first and last name, address, phone number, assigned member number, and the date the membership was purchased. Please include a reason for cancellation. The email must be sent within 30 days after the effective date of enrollment for your refund to be valid. 

Your membership will automatically renew one year from your enrollment date unless otherwise cancelled. You will need to send an email to info@careultima.com within 5 business days before the renewal date in order for your membership to cancelled before the billing date.  Send an email to info@careultima.com and provide the members first and last name, address, phone number, assigned member number, and the date the membership was purchased.
